Role Overview

The Social Worker will report directly to the Social Work Coordinator and will be responsible for providing necessary support to patients and their families, coordinating services, and contributing to holistic, patient-centred care. The role includes assessment, care planning, and support provision, including bereavement support, and collaboration with healthcare professionals and community services.

Key Duties and Responsibilities

  • Conduct necessary assessments of patients and families
  • Provide emotional, practical, and social support to patients and families during illness, including the provision of bereavement support
  • Participate in multidisciplinary team meetings and collaborate with internal and external professionals
  • Maintain accurate and confidential records of assessments, visits, and follow-ups
  • Liaise with community services and make referrals to appropriate agencies as necessary
  • Coordinate and facilitate support groups and assist in the delivery of bereavement support programmes
  • Promote awareness of palliative care within the community
  • Use organisational resources responsibly while maintaining high standards of care
  • Practice in accordance with the Code of Ethics issued by the Malta Council for the Social Work Profession (MCSWP)

Qualifications & Experience

  • Qualification in Social Work (Diploma or Bachelor’s degree in Social Work)
  • Proficiency in both Maltese and English languages is essential
  • Valid and clean driving licence
  • Registration with, or eligibility for registration with, the Social Work Profession Board in Malta
  • Experience in social work practice will be considered an asset. Applications from recently qualified social workers will also be given due consideration.
