SUPPORT PROFESSIONAL CARE AT THE PATIENTS’ HOME

Home care is one  of Hospice’s main services.  Each patient  is allocated a key nurse who oversees their care and is responsible  for liaising with the Care Team members and other relevant agencies.  Patient/family benefits from this service:

  • Patients and families receive professional advice/home sessions from nurses, doctors, social workers, physiotherapists, psychologist, complementary therapist, chaplain  as well  as care assistance which is tailor made for each patient’s individual needs.
  • An assessment for pain and symptom control, and for the person’s psychological, social and spiritual needs.
  • An individualised care plan which is reviewed and amended  by the professionals on a regular  basis, according to the patient’s ongoing needs.
  • Ongoing liaison with the patient’s family/relatives.

SUPPORT THE CARE ASSISTANT SERVICES

Care Assistants support patients in their own home. The aim is to reduce the care giver’s burden and empower the families to care for the patient at home as long as possible. Each year, 29,000 hours of care are being delivered.

SUPPORT THE DAY THERAPY AND HYDRO THERAPY SERVICES

This  service  is run  by  the  Day Therapy Unit staff  with  the  help  of volunteers.  Patients have access to various therapeutic sessions including physiotherapy, complimentary therapy, hydro therapy, spiritual and emotional support.  Social activities are organised on a regular  basis including outings, preparing and sharing meals, amongst others, thus preventing social isolation as well providing respite support for relatives/family members.

Throughout the year, around 150 families benefit from this service.

LOAN OF EQUIPMENT

Hospice owns  an inventory of equipment that is offered on loan to patients.  These include  fully motorised beds, lifting equipment, wheelchairs and mobility equipment, respiratory and toileting equipment, amongst others.  These are loaned free of charge to patients following  a detailed assessment of their needs.   This greatly improves the independence of patients and saves them the cost of purchasing the equipment.

YOUR €1,500 donation helps support 10 patients.

SUPPORT THE BEREAVEMENT SUPPORT SERVICE

 The Bereavement service is available to relatives/significant others who have lost a loved to one of the diseases  included  in the admission  criteria.  The Bereavement Care Team consists of the hospice social workers and psychologist  trained in bereavement care under  the  supervision  of the  care  Services  Manager.  The first post death  contact  is initially provided by the key worker, with referral to the Bereavement  team or other professional agencies   as  appropriate.   The Bereavement   care services   consist of  support  groups, individual sessions, monthly Memorial Mass and Annual remembrance Event

YOUR €500 donation will support 10 bereavement sessions.

SUPPORT THE AFTER HOURS ON-CALL SERVICE

Ensuring continuity of care, Hospice has introduced an after-hours on-call service to patients 7 days a week (weekdays till 10:30pm and Weekends/Public Holidays from 8am till 10:30pm).  Patients and relatives receive nursing and practical advice, information and support.    Throughout the year, Hospice receives close to 400 calls for this service and requires approximately €11,000 per annum to cover the costs.

YOUR €270 donation will support 10 calls.

SUPPORT PATIENT TRANSPORT

Delivered through special vans for wheelchair mobile patients. One van is used solely for transporting of oncology hospital patients. More than 200 patients make use of this service every year.  Help us sustain this service by donating €2 per month.
