SUPPORT PROFESSIONAL CARE AT THE PATIENTS’ HOME

Home care is one  of Hospice’s main services.  Each patient  is allocated a key nurse who oversees their care and is responsible  for liaising with the Care Team members and other relevant agencies.  Patient/family benefits from this service:

  • Patients and families receive professional advice/home sessions from nurses, doctors, social workers, physiotherapists, psychologist, complementary therapist, chaplain  as well  as care assistance which is tailor made for each patient’s individual needs.
  • An assessment for pain and symptom control, and for the person’s psychological, social and spiritual needs.
  • An individualised care plan which is reviewed and amended  by the professionals on a regular  basis, according to the patient’s ongoing needs.
  • Ongoing liaison with the patient’s family/relatives.
